Appointments
Cancellation & No-Show Policy
We request at least 24 hours’ notice for cancellations or rescheduling.
Late cancellations or missed appointments may result in a fee of $75.00
Repeated no-shows may result in discharge from the practice.
Prescription Refill Policy
Medication refill requests should be submitted during regular business hours with adequate notice.
Refills are generally addressed during scheduled appointments. Controlled substances may require follow-up visits before renewal.
Communication Policy
For routine questions, please use the patient portal or office contact methods. Messages are reviewed during business hours.
Do not use email or portal messaging for emergencies.
Emergency Policy
Apex Mind Health Solutions is an outpatient practice and does not provide emergency or after-hours crisis services.
If you are experiencing thoughts of harming yourself or others, call 911, go to the nearest emergency room, or dial 988 immediately.
Payment Policy
Co-pays, deductibles, and self-pay balances are due at the time of service unless other arrangements are made.
Patients are responsible for understanding their insurance benefits.
Confidentiality
Your privacy is important to us. We follow HIPAA standards and applicable laws to protect your health information.
Information may only be released with written consent or when legally required.
Controlled Substance Policy (Recommended)
Controlled medications are prescribed only when clinically appropriate and may require:
- Ongoing follow-up appointments
- Review of prescription monitoring databases
- Urine drug screening when indicated
- Adherence to treatment agreements
